To me it meant that Michael had let them out when he got out as a distraction while he made his escape. I mean it would be hard to keep track of Michael specifically when all those patients are running around too. That or he had killed some of the staff and that left the place unsecured and some of the patients that weren't in their rooms were able to just wander off.
